| Due
date of first monthly payment is based on the effective date of
the policy. Example: Policy effective 3/14/01, the
first monthly payment is due 4/14/01. |

| The
Premium Finance Contract must be signed by the named insured.
If there are two named insureds, the first named insured listed on
the application/policy must sign. If the insured is
incorporated, an officer of the company can sign as long as they
list their title with the corporation beside their signature. |

| SIPP
distributes a coupon booklet to the insured after the contract is
processed in our department in lieu of billing each month. |
|
| Example:
If the contract is on the 8 pay plan, the insured will receive a
thank you letter along with 8 coupons including 8 labels to remit
payment to SIPP. |
